Total Joint to Outpatient Program (TJOP)
Our Total Joint to Outpatient Program (TJOP) is designed to offer comprehensive pre- and post-operative care for patients undergoing total joint replacement. The program includes home therapy, nurse practitioner visits, physical therapy, medical management, falls education, and screening for a successful transition from hospital to home and outpatient therapy. TJOP is tailored specifically to meet the needs of total joint patients as they move through each phase of recovery.

Your Dedicated Care Team
Each patient in the TJOP program is supported by a dedicated care team. The team includes a Team Leader—either a licensed physical or occupational therapist—and a Nurse Practitioner. The Team Leader’s role is to assess your clinical needs, ensure the program is tailored to your recovery, and lead therapy sessions for 3-4 weeks. The Nurse Practitioner manages your medical care throughout the entire recovery process.
The collaboration between Team Leaders and Nurse Practitioners is crucial to your success. They maintain regular communication with you and your family, providing ongoing assessments to track your progress toward full recovery and independence. By building a strong, supportive relationship with both you and your loved ones, our team fosters a commitment to a safe and successful recovery.
