The Total Joint to Outpatient Program (TJOP) is a program aimed at providing pre-and post operative treatment for total joint patients including in home therapy and nurse practitioner visits, physical therapy, evaluation, medical management, falls education, screening, and intervention for a successful transition from the hospital to home to outpatient therapy. The TJOP program will be utilized to treat the total joint patients directly from the hospital.

Our clinicians are driven to constantly improve their knowledge and the services they provide, our staff frequently attend conferences and society meetings to learn about the latest advancements in the field and offer these techniques and technologies to our patients. In all of these ways and more, we are able to provide our patients with the highest quality of care.

The Importance of Patient Education

A key part of preventing a health care problem from progressing is being aware as to how they can develop. We strongly believe that patient education is an important part of the services we provide. In addition to talking with our patients during their exams, we have a ready supply of patient education materials such as brochures and information sheets we provide. We encourage our patients to read about their surgical conditions and ask us questions.

Patients have their own team of clinicians

This program is implemented with a team approach. Every team has a “Team Leader” and a “Nurse Practitioner”. The Team Leader is a Licensed Physical or Occupational Therapist. The purpose of the Team Leader role is to assess the clinical needs of the total joint patients and ensure the program is appropriate for each potential surgical patient. Team Leaders will be involved with every session and will conduct periodic clinical assessments to determine progress towards full recovery and independence.

The Team Leaders and Nurse Practitioners are responsible for coordinating the implementation and weekly details of the TJOP Program. Team Leaders run every therapy session for 3-4 weeks and serve as the primary contact for the patient. The Nurse Practitioner medically manages the patient through their full episode of care. There will be regular communication between the Team Leaders and the Nurse Practitioners throughout the full patient recovery. It is critical to the success of the TJOP program that the Team Leader and Nurse Practitioner build a strong relationship with the patient and the patient’s family to foster commitment to improving a successful and safe recovery.
